Amateur Team Athletic Waiver & Release of Liability Form and
the Athletic Registration Form are both in .pdf format. Adobe
Acrobat Reader is required to view and print these forms - if
you do not have Adobe Acrobat Reader then a link has been provided
so that you will be able to acquire it. Once you have printed
up the forms, please fill them out and send them along with your
check to the address indicated on the forms.
In
the future we hope to give you the option to fill out, submit
and pay on-line, thus ushering us into the new technological era;
unfortunately, for now, we will have to use the 'traditional'
method.
